The Bair Foundation – Coaching Intake Form

This coaching intake form is for Bair Foundation staff to complete when they have a family in need of parent coaching. Completing this form commits the family to six coaching sessions. Five of the sessions will be with the family and their Empowered Parent Coach. The sixth session will include the family’s Case Manager to ensure that everyone is on the same page moving forward.

Once the intake form is completed, you will be redirected to a payment page. Once payment has been submitted, you will be redirected to a coaching calendar link which you can share with the family so they can schedule their first session. 

This link will be used for each subsequent session, or the family can opt to schedule at a regular time (ie: six Wednesdays at 11am) and we will create a recurring event.
